Update existing forms #
Each Perceptions Sites template has several forms already configured. You can find the listing by navigating to Forms in the Dashboard navigation and clicking the All Forms link.

Click on the form you wish to update. This will bring up all form options, including the ability to add or edit form fields, the ability to change the setup, add email notifications, as well as edit form messages and styling.
Add email notification when a form is submitted #
To configure emails, navigate to Email tab by clicking it in the navigation at the top of form builder.

You’ll find a number of important settings you’ll want to fill out so your form submissions are delivered correctly. At the very least, you’ll want to review these two settings:
- Email me a copy of each submission — receive an email alert with a copy of the data your users submitted through your form.
- Email respondent a copy of their submission — send a confirmation email to your users after they submit your form.
Each of those settings reveal a number of related controls once checked. You can configure a ‘Reply-To’ address here, send a copy of submitted data to your co-workers using Email Bcc address control, or even change email subject based on the submitted data by adding a logic group!
If you don’t want to send emails to users or receive alerts, clicking appropriate checkbox will hide the settings and disable that feature.
Access form submissions #
You can access form submissions by going to Forms in the Dashboard navigation and then clicking Activity. This will give you a list of all submissions through all forms.

To download a copy of submissions, click Settings in the Forms menu. Then in the dropdown, select “Export submissions to a spreadsheet.”
